Shouldn't it be:
snmp_acl allow mrtgsnmp localhost
snmp_acl deny all !localhost

instead of snmp_access

Doesn't it miss
snmp_mib_path /usr/local/squid/etc/squidmib.txt

And just in case:
snmp_port 3401
