Most on this list are using a package called PerfM.Zip that is available on
ftp.shellserv.com.  This is the collection of files from the NT ResKit that
exposes PerfMon counters via SNMP.  The instructions are included. 

If you need more info, check the list archives as this topic has been
discussed many times in considerable depth.
