You want to look at mrtg-ping-probe in the contrib directory of the 
mrtg-2.8.x source.  (not sure baout 2.9.0preX)  

I use it to ping the first upstream of my cable connection and of a 
few routers scattered around the world at my place of employment.  It's 
fairly easy to use and has a sample config with it.
