I think the answer to the first question is to just repeat the string
because OID at xxx + OID at xxx yields one number, MRTG needs two.  Like this:

Target[TOTAL-MODEMS]: 1.3.6.1.4.1.9.9.47.1.1.6.0:guronet at POU-NAS1  +
1.3.6.1.4.1.9.9.47.1.1.6.0:guronet at POU-NAS2&1.3.6.1.4.1.9.9.47.1.1.6.0:guron
et at POU-NAS1  +
1.3.6.1.4.1.9.9.47.1.1.6.0:guronet at POU-NAS2

The Cisco 5100 question sounds similar to the question I passed to Cisco on
my AS-5200.  According to the documentation the mib should work but when I
talked to Cisco about the failure with 11.2(17) code they said I would have
to upgrade.  It seems strange to me that you can query the number of unknown
protocols on an interface and a million other exotic stats but you cannot
get the number of modems in use.

can we capture the modem usage at Cisco Access Server 5100 with IOS
11.2(18)? because i got 'Received SNMP response with error code' and
'SNMPGET: Failed to reach target' am i missing something?
