I don't believe MRTG has this ability however Big Brother can do 
something like this.

I have a number of routers/switches in which Big Brother pings every few 
minutes.  It's running on a Sparc with Solaris 2.8.  Any device that 
goes down will generate an email within minutes.

If you are looking for specific ports that go from an up to a down 
status then Big Brother "should" be able to accomodate that also.  I 
haven't done this myself but they have SNMP support.

They have a mail list you can join for the details.
