The instructions for template files can be found in the cfgmaker manual.

You don't replace the variables, you use them to specify to cfgmaker what
you want to put in the variables' place, eg $ifindex will be the ifIndex
value for that target, $router_name will be the host name of the box you're
polling. I found it pretty easy to figure out what each variable does just
by dinking around with the template, plus there are example templates at the
bottom of the page.
To use a template, you just use the --if-template=[name of your template
file], as it says in the cfgmaker help.
Note that this option is labeled (Experimental, under development, might
change)
