I don't know if there is a tool, however, it can be done with
a simple text editor:
- dump the content of the database with rrdtool dump
- edit the xml file in the correct way (this is not too hard
  to figure out)
- reload the database using rrdtool restore

First try the restore to another name until you know everything
is well.
