Make sure that the libraries rrdtool needs are in the default library
path; you can get the list of libraries using "ldd rrdtool.so".

I've beat my head on this repeatedly myself.  I ultimately finally just
made a distribution of php, rrd, apache, etc with hard coded paths
pointing to a single /opt/packagenam with *everything* in that tree.
Got it working after some pain.
