Had the same problem with the disk on some digital servers.  I have
installed some new compaq server with raid and they are ok.  I can monitor
all logical drives configured on the compaq raid controller.  Do an snmpwalk
form the disk oid to ensure that you have the correct oid for the d drive.
NT and OID's can be different depending on hardware config (and time of day
for that matter :-)).  If you can get the oid from a walk then you should be
able to monitor.  As for the CPU, do what I did.  Forget about getting stats
for both.

After reading a number of archived posts about monitoring CPU/disk
activity on NT boxes I expanded the SNMP variables on my NT
box(perfmib.zip & GETif). I can properly graph free space on drive c:
but I cannot get any stats for drive d: (raid-array).

I'm using this OID for drive D:
1.3.6.1.4.1.311.1.1.3.1.1.6.1.1.6.48.58.48.58.67.58.0&1.3.6.1.4.1.311.1
.1.3.1.1.6.1.1.6.48.58.48.58.67.58.0:public at xxxxxxx

I'm also trying to graph CPU utilization but always receive a 100%
value when using:
1.3.6.1.4.1.311.1.1.3.1.1.17.1.2.0&1.3.6.1.4.1.311.1.1.3.1.1.17.1.10.0:
public at xxxx
I have read that you must use an OID pair that has one value that will
always be 0 to offset and bug in the IP stack implmentation on NT
boxes. Still, I get 100% or 0% in the case above. 
Any help would be greatly appreciated.
