Are you running through the firewall? If so, have a look at the following 
document:

http://www.cisco.com/warp/public/110/pixsnmp.html

and search for "SNMP through the PIX" to ensure that the PIX is correctly 
configured to allow 161/udp (and 162/udp for traps) through.
