[mrtg] Switch Ports

Is there a way to determine what server IP's are on what port of a switch?  We colocate our servers and think they are providing us with the wrong IP's for the wrong ports.  

Another question along the same lines.  Would you recommend monitoring servers individually with SNMP on each server or monitor them from ports of the switch?  Is the only reason you wouldn't want to monitor by ports on a switch when you don't have a swich?  Thanks!
