Matt,
you have very good points here, and I will definitely look closer into
the  Service Assurance Agent;  if it can be queried through SNMP, it
should be easy to integrate into mrtg or smokeping.

there is also the Cisco IOS RTR (Response Time Reporter)

some people also proposed to use your ntp servers, they keep track of
the delay between them.
