It can be some unicast traffic, which ends on the router.
For example ICMP ping to router.
Or maybe some huge broadcast or multicast traffic, which is not distributed
to other networks. I don't know now if broadcast or multicast traffic is
countered in to IfOutOctets.
If you can't use ethereal or similar packet grabber/analyzer, you can
enable on router IP accounting or NetFlow.

Example:

interface Ethernet0
 ip accounting output-packets

#show ip accounting
----------

interface Ethernet0
 ip route-cache flow

#show ip cache flow
----------
