A preliminary result that was easy to achieve and fun to do:

(the blue poles seem one pixel off to the left, I'll look into that too)

So far, I made changes to:
  rrd_graph.c (added "yaxis-side" parameter, changed+added some code)
  rrd_tool.c (enhanced help_graph2[])
  rrd_graph.h (enhanced image_desc_t)
