Yes. You need to do it for every 'step' value (or at
least by the heartbeat).
Guess APAN and Nagios are rrdtool frontends, never used
them though. Anyway check if it's updating right with 
'rrdtool fetch [RRD_DB.rrd] --start [...] [CF].
Also you can do some test graphs to make sure it's OK.
