Most of this can be found in the manual.

In the cfg use the MaxBytes tag and set this as your CIR.  Calculations
will be based on this eg % utilisation.
Use AbsMax as your burst.  This will allow the graphing of any data over
the CIR.

Remember though the values for these are bytes not bits.

As for q2, you can have a different cfg for every device.  This is what
I do.  Makes it easier to add/remove devices from monitoring

Your command line (under winnt) will look something like this 

Perl mrtg configfile1.cfg

I run mine in a for-in-do loop, catches all of the cfg files for
particular device types.

Q3 though will require a little more effort.  If the value returned by
the device for the PVC status is 0 or 1 then you can graph that using
mrtg.  Will graph when the interface was up or down but wont really tell
you exactly when it was up or down, just tell you since the last polling
period.  The graph will ook pretty ordinary though.  You could look at
using the threshold alerting to alert you when the status changed, but
again only since the last polling period.
